Supporting the delivery of community-based diabetes education, this role helps individuals develop the skills and confidence to self-manage their condition. Working alongside a multidisciplinary team, you will deliver engaging group and one-to-one sessions, promote healthier lifestyle choices, and improve access to education across local communities.  You will also support referral pathways, monitor outcomes, and contribute to service development—playing a key role in improving diabetes care and patient wellbeing across the region.

### Experience

**Essential**

- Experience of meeting individuals with diabetes educational needs
- Experience of working in a multidisciplinary team

**Desirable**

- Knowledge of sign posting to health care & lifestyle services
- Experience of coordinating programs of care or service delivery

### Qualifications

**Essential**

- Health/Social care experience NVQ Level 4
- Microsoft 365 applications
