This is an exciting opportunity for someone who is passionate about improving health and wellbeing, reducing health inequalities and making a genuine difference to patients and the wider community. You will be joining a forward-thinking practice where we value teamwork, collaboration and patient-centred care. The role offers variety and the opportunity to work with people from a wide range of backgrounds, both within the practice and across the wider community. You will work alongside an established multidisciplinary team and build relationships with patients, local organisations and community partners. We are looking for someone who is approachable, enthusiastic, flexible and confident working with people, with a genuine interest in helping individuals improve their health and wellbeing. If you enjoy working as part of a team, but are also comfortable working independently and taking initiative, we would love to hear from you. This is an ARRS-funded role, with flexible hours and pay dependent on experience. ### Main duties of the job Key Responsibilities Personalised behaviourchange coaching Provide structured onetoone coaching to help patients identify goals, build confidence, and make sustainable lifestyle changes. Practicebased patient support Deliver facetoface sessions at Elm Tree Medical Centre and other sites, including wellbeing assessments, care planning, and followup reviews. Communitybased engagement Work offsite across local venues, voluntary organisations, and social groups to connect patients with appropriate community resources. Group education programmes Facilitate group sessions on topics such as healthy living, physical activity, stress management, and social connection. Integrated multidisciplinary working Collaborate with GPs, nurses, social prescribers, mental health practitioners, and other allied health professionals to ensure coordinated patient care. Person Specification Demonstrable commitment to improving health outcomes and reducing health inequalities. Strong communication skills with the ability to motivate, encourage, and build rapport with patients. Experience working with diverse communities and individuals with varying levels of need.
