### Job summary

Health and Wellbeing Coaches support patients to take pro-active steps to improve the way they manage their physical and mental health conditions, based on what matters to them. They support patients to develop their knowledge, skills, and confidence in managing their health and care, to improve their health outcomes and quality of life, and support them in making changes to their health-related behaviours. Health and Wellbeing Coaches do this by coaching and motivating patients through multiple sessions to identify their needs and to set goals for themselves, and through providing access to interventions such as self-management education and peer support.

Health and Wellbeing Coaches manage and prioritise a caseload, according to the needs, priorities and support required by patients in the caseload. They identify when a patient's needs are beyond the scope of the Health and Wellbeing Coach role and refer them back to other health professionals or organisations as appropriate. Health and Wellbeing Coaches may work with patients by phone, by video conference, or face-to-face.

The successful candidate will be based at Castle Health Centre as part of the SCORE Primary Care Network (PCN). They will be caring, dedicated, reliable and patient-focussed and enjoy working with a wide range of people.

### Main duties of the job

Clinical Practice

To ensure that all statutory requirements outlined in the IntraHealth personnel procedures and policies are personally adhered to.

To be familiar with and conform to responsibilities under the Data Protection Act as identified by IntraHealth.

To undertake duties as necessary, in line with the changing needs of the company. To participate in the annual appraisal and knowledge and skills framework profiling process.

Acts in a way that acknowledge patients rights to make their own decision and recognise their responsibilities.

Facilitates others to identify their current level of knowledge and skills, their learning needs and best practice.

To support others in the development and application of knowledge and skills in practice.

Identify resource issues which affect learning, development and performance, and alert the appropriate managers.

### Job responsibilities

Overview

Health and Wellbeing Coaches work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed. This is achieved by bringing together all the information about a patient's identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the patient

Health and Wellbeing Coaches review patients' needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to other professionals where appropriate

Health and Wellbeing Coaches provide personalised, holistic support to patients within primary care, explore ways of improving mental health and wellbeing

Health and Wellbeing Coaches support patients to utilise self-help and facilitated tools to aid individual goal planning and measurement of progression

Health and Wellbeing Coaches support patients to make informed choices about their own health and wellbeing, which includes (but is not limited to) improving the levels of physical activity, promoting a healthier diet, and accessing self-help tools

Health and Wellbeing Coaches effectively signpost and encourage participation to appropriate local services

Health and Wellbeing Coaches share good practice within the surgery and the wider PCN, promoting innovative approaches with a view to improving health outcomes

### Experience

**Essential**

- Experience of working in Primary Care with direct contact with patients, families, and/or carers
- Proficient in the use of IT systems (Microsoft Word and Excel) to a level to enable the production of high standard of work
- Awareness of health inequalities and experience of working with diverse and underserved populations
- Proficiency with handling patient data and using digital systems and technology

**Desirable**

- Experience working on clinical computer systems - SystmOne
- Knowledge of Safeguarding Children and Vulnerable Adults policies and processes
- Understanding behaviour change theories such as Motivational Interviewing or CBT-based approaches
- Experience of supporting patients with lifestyle modification e.g., weight loss management, physical activity, smoking cessation
- Understanding of Type 2 diabetes, hypertension, anxiety & depression, COPD, and SUD (substance use disorder)
