### Job overview

The Torquay Intermediate Care team have a  short term vacancy for a 18.75 hour  maternity leave cover Health and Social Care Co-ordinator working Wednesday and Fridays. The may be need for Occasional weekend cover.

Working in a team providing a single point of contact for community health and social care referrals.  This will include talking to a variety of individuals over the phone, via emails and face to face.   Collation of appropriate information to record a needs led referral or assessment,  which will include applying Care Act Eligibility and identifying potential risk.  To provide advice, information and signpost to other local statutory and voluntary services as well as completing full Care Act assessments of need and formulating Support Plans and acting as a Keyworker.

You should have excellent communication & I.T. skills, have a knowledge and understanding of the Care Act Legislation & Eligibility and be able to multi task and prioritise effectively.   Previous experience working in a Health and Social Care setting with Patients/service users and other members of the public is necessary as is the ability to write up documents and reports clearly and concisely.

### Main duties of the job

- To provide a consistent point of contact for members of the public or professionals. To undertake conversations to identify needs and risk and collate accurate and appropriate information to refer to the multi- disciplinary team or other Community Led and Voluntary/ Statutory services.   To carry out social care assessments, apply eligibility  and contribute to multi-disciplinary assessments in accordance to the Care Act 2014. To Provide advice and information where appropriate and to provide short-term management of a caseload as well as formulating support plans.
- To co-ordinate a caseload working alongside a multi-disciplinary Team
- To research a variety or IT solutions
- To undertake Social care assessments ensuring you work within a legislative framework including the Care Act
- To commission appropriate Health and Social Care Services in accordance with current eligibility and liaise with Service Users, carers, families and colleagues
- To formulate/contribute to detailed support plans/summary of needs and contribute to setting goals and objectives based on assessments carried out.

- To liaise with individuals who may have communication difficulties due to a speech and language needs, a sensory impairment or a mental health need.
- To exercise tact, empathy & sensitivity at all times and use negotiation skills.
- To liaise and work closely with other members of the multi-disciplinary team
- To communicate medical, social & sensitive information ensuring General Data Protection Regulations are adhered to.
- To be able to work in an extremely demanding, pressured and stressful environment with constant interruptions by telephone, colleagues and professional office visitors, all relating to current cases or new enquiries.
- To be able to multi-task, re-prioritise your workload at short notice and identify risks and urgent work that requires action.

- To work pro-actively to take referrals, manage a small caseload & co-ordinate a support plan to meet identified needs.

- Recent experience working in a health or social care setting with patient contact
- Experience of multi-disciplinary working
