### Job overview

We are currently seeking health and social care assistants with a broad range of previous experience and knowledge who are looking for an opportunity to develop skills in rapid response and management of acutely unwell clients in the community. You will be working under the guidance of multi-disciplinary clinicians, to support assessment and treatment of patients’ to achieve medical stability and maintain functional ability, aiming to prevent unnecessary hospital admissions.

### Main duties of the job

You should have previous experience of working with a therapy, nursing or personal care team within a health or social care setting. You must be flexible, responsive and have excellent time management skills as the teams priorities can change throughout the day. You should have good written and verbal communication skills and enjoy working in a dynamic and effective team. As the service is community based, you will need your own means of transport and will be required to work long days (7am to 7pm) and weekends on a rota basis.

### Applicant requirements

## Person Specification

### Knowledge

**Essential**

- Understanding of professional conduct
- Basic understanding of the effects of disability
- Health and safety / risk awareness
- Understanding of concept of enabling clients through the therapy process
- Familiar with equipment to assist activities of daily living

### Experience

**Essential**

- Previous work in health or social care setting (or above essential qualifications)
- Experience of working in paid or voluntary employment or community work
- Previous role as Rehabilitation Assistant / Health Care / Social Care Assistant

### Qualifications

**Essential**

- NVQ in health or social care (or below essential experience)

**Desirable**

- Care Certificate for Support Worker
- Foundation Degree in related subject
- Trusted Assessor

### Skills and Abilities

**Essential**

- Good written and verbal communication
- Ability to work as an effective team member
- Fully licensed car driver or independently able to travel to manage an agreed schedule of community appointments
- Ability to comply with moving and handling regulations
- Ability to work flexible hours including weekends
- Competent IT skills to enable effective documentation
