The role involves providing culturally appropriate education, supporting self-management, promoting healthy lifestyles, facilitating engagement with healthcare services, and working collaboratively with healthcare professionals, community organisations, schools and universities, and voluntary agencies. Key deliverables will include: - Providing health education and self-management advice aimed at preventing exacerbation of the condition and minimising admissions for acute episodes to secondary care. - Addressing social and lifestyle factors that impact condition management and overall quality of life, signposting to local resources when required. - Building trust and rapport with patients whilst maintaining professional boundaries. - Providing an In-reach service to secondary care. - Developing close links with multidisciplinary colleagues across primary, secondary, and social care to enhance continuity of care for patients. - Organisation of patient led education sessions and involvement with peer support programmes. ### Detailed job description and main responsibilities Please view the attached Job Description and Person Specification documents for full details regarding this post.
