This role will be involved, as part of the wider team, in helping to adapt the existing service within the new transformation process. This process is in response to the NHS 5-year plan to develop mental health services round the Primary Care Networks and provides new opportunities which include developing partnership working with the voluntary sector, working more closely with GP practices and developing more innovative, community based and holistic ways of working. The post holder will be responsible for participating and assisting in the delivery of care/services to patients, service users and carers as part of the Core Community Mental Health Team, working under the indirect supervision of a registered member of staff. You will be responsible for carrying out practical tasks, identified as part of the service users’ care plan or care pathways. The post holder will provide supportive care management to an allocated group of service users as delegated by the Team Manager or registered practitioner. The post holder will receive clinical supervision as appropriate and partake in reflective practice/training on a regular basis. ### Main duties of the job Undertake a range of duties which involve direct patient care, in some cases under supervision following competency assessment. Assist in the coordination of care delivery for a case load of service users in the community with mental health problems including those with severe and enduring mental illness under the supervision of a registered professional, in close collaboration with the Directorate, psychiatric multidisciplinary team, service users and other community agencies. Work under the supervision of qualified staff that supervise their caseload but they work independently with their service users. Contribute to the monitoring of the physical environment ensuring that it is safe and conducive to the therapeutic work of the unit/community setting. Contribute towards the delivery of care using the recovery approach to provide therapeutic interventions under supervision by means of assessment and ongoing treatment in various settings. Design care programmes in line with risk assessment and psychosocial needs. Utilise designated recovery tool with outcomes focusing on care planning, ensuring clients understand they will be worked with in a collaborative manner to engage in social inclusion activities such as seeking employment, education and independent living To be conversant with factors that cause mental illness and preferred treatment methods Support the assessment and management of client and carers needs through initial data collection, monitoring of patients’ progress, feedback, discussion, and reports and in care reviews with the clinical team. Assist in the physical health care assessment of service users and report any changes in service users’ physical health to the registered practitioner Provide information of health promotion and healthy living Take part in psychosocial interventions i.e. CBT, psychoeducation or family work as per service user care plan.
