If it is identified that you require sponsorship to undertake this role your application may be withdrawn As a Graduate Mental Health Worker you will assist in the assessment of care needs and the formulation, implementation and evaluation of programmes of care for individuals. You will deliver relevant forms of psychosocial and psychologically informed care under agreed levels of supervision. You will assist in identifying strategies to support mental wellbeing and in the promotion of self-care and social inclusion. You will communicate and work collaboratively with Mental Health team colleagues and the prison teams to meet the needs of patients. You will support the delivery of timely access to integrated, high quality, service user-centred services that enable people with mental health needs, and/or substance use issues to improve their well-being and functionality, thereby preventing, reducing or delaying the need for more specialist services. As part of the integrated team, under the supervision of qualified mental health practitioners, you will work to ensure that the needs of service users are addressed by offering appropriate interventions in both one to one and group settings. This will include promoting wellbeing, independence and choice whilst preventing, reducing or delaying the greater onset of need. You will be expected to adhere to relevant policy and procedure. You will liaise with other professionals and agencies both statutory and non-statutory to provide holistic care and treatment. The co-facilitation of group work will be an expectation of the role.
