We are expanding our Intermediate Tier Services and seeking motivated GPs to join our Intermediate Care and Hospital at Home teams. Roles are part‑time, with flexible PAs agreed at interview. The Stamford Unit provides intermediate care beds offering rehabilitation, end‑of‑life care, step‑up admissions and complex discharge planning. You will lead the medical care for a cohort of patients, undertake ward rounds and work closely with ACPs, Nurses, Therapists and Social Workers to deliver safe, holistic care. Hospital at Home delivers acute‑level care in patients’ homes as an alternative to admission. Working with Urgent Crisis Response teams, advanced nurse and paramedic assessors and specialist respiratory and heart‑failure services, you will complete telemedicine assessments and support coordinated, responsive care planning. This varied role includes both bed‑based and community acute work, with the chance to develop interests in older people’s medicine and integrated care.
