The successful candidate will support our GPs and nursing team in delivering safe, high-quality, person-centred care, while helping coordinate patient care and promote health and wellbeing. You will be approachable, organised, an excellent communicator and able to work effectively both independently and as part of a multidisciplinary team in a busy primary care environment. The role is for approximately 20 hours per week, with additional hours available to cover annual leave and sickness. You must be willing to work across all practice sites and participate in a rota covering 46 Saturday mornings per year, with adjusted weekday hours. The post holder will undertake delegated clinical duties within their competence, support the delivery of preventative healthcare programmes, assist with chronic disease monitoring where appropriate, and contribute to the smooth running of clinical services. The role also includes care coordination responsibilities, helping to ensure patients receive timely access to appropriate services, supporting continuity of care, and working closely with the wider multidisciplinary team. As the Practice Group operates from multiple sites, the successful candidate will be expected to work across all locations as required.
