### Job summary

We are looking to recruit a motivated, compassionate and enthusiastic Healthcare Assistant / Care Co-ordinator to join our friendly, award-winning multi-site GP practice team.

The successful candidate will support our GPs and nursing team in delivering safe, high-quality, person-centred care, while helping coordinate patient care and promote health and wellbeing. You will be approachable, organised, an excellent communicator and able to work effectively both independently and as part of a multidisciplinary team in a busy primary care environment.

The role is for approximately 20 hours per week, with additional hours available to cover annual leave and sickness. You must be willing to work across all practice sites and participate in a rota covering 46 Saturday mornings per year, with adjusted weekday hours.

### Main duties of the job

Following appropriate training and assessment of competence, the successful candidate will undertake, record and work within agreed clinical guidelines for duties including:

- Phlebotomy (mandatory)
- Blood pressure monitoring
- Pulse rate and rhythm assessment
- Temperature measurement
- Height, weight and Body Mass Index (BMI) recording
- Basic wound dressings
- Electrocardiograms (ECGs)
- Administration of seasonal influenza, RSV, Shingles, Pneumonia vaccinations (where trained, or training will be provided)

In addition, the post holder will:

- Prepare and maintain clinical environments and equipment before, during and after patient care interventions.
- Support the delivery of high-quality clinical services within the practice.
- Assist in promoting health, wellbeing and healthy lifestyle choices to patients.
- Provide accurate and appropriate health information to patients within the limits of their competence.
- Support care coordination activities to help ensure patients receive timely, appropriate care and follow-up.
- Work collaboratively with the wider multidisciplinary team to deliver excellent patient care, including our local PCN
- Maintain accurate clinical records in accordance with practice policies and professional standards.

### Job responsibilities

The Healthcare Assistant / Care Co-ordinator will work under the direction of the GPs and nursing team to support the delivery of high-quality patient care across the practice group.

The post holder will undertake delegated clinical duties within their competence, support the delivery of preventative healthcare programmes, assist with chronic disease monitoring where appropriate, and contribute to the smooth running of clinical services.

The role also includes care coordination responsibilities, helping to ensure patients receive timely access to appropriate services, supporting continuity of care, and working closely with the wider multidisciplinary team.

As the Practice Group operates from multiple sites, the successful candidate will be expected to work across all locations as required.

Undertake delegated clinical procedures following appropriate training and competency assessment, including:

- Phlebotomy (essential)
- Blood pressure monitoring
- Pulse rate and rhythm
- Temperature measurement
- Height, weight and BMI recording
- Urinalysis
- ECG recording
- Basic wound care and dressings
- Seasonal influenza, RSV, Shingles, Pneumonia vaccinations (where trained or training will be provided)
- Collection and processing of specimens
- Chaperoning clinicians where appropriate
