As a Generic worker within a community based integrated team, you will have the opportunity to support safe, effective care. You will work alongside the nursing and therapy teams with registered practitioners to support specific tasks as part of the individuals' care plan. You will have a mindset to promote independence for activities of daily living and encourage self-care wherever possible.  You will carry out routine elements of assessment, work to care plans, and carry out delegated care/therapy interventions. You will also work alongside our social care practitioners, care-coordinator, community and voluntary partners utilising an MDT to achieve best outcomes for our patients. Our supportive team will support you to develop skills, and knowledge about an integrated health and social care response to care in the community, in response to NHS plans for place-based, proactive and preventative care and digital developments.
