Are you an enthusiastic and motivated individual looking for a new role that will challenge and develop you? Do you want to join the Pulmonary Rehabilitation service in a supportive role?

The Pulmonary Rehabilitation team includes Physiotherapy, Occupational Therapy, Dietetics and respiratory Nursing. The service supports people presenting with chronic respiratory disease. The team work closely with the Adferiad Service . Your job will involve both clinical and administrative duties to facilitate the service.

The post holder will be required to travel/undertake clinical work across the Swansea Bay UHB area to support service delivery working directly and indirectly with patients and families in outpatient clinics/ community groups and in their own homes when necessary and staff members.

### Main duties of the job

Be a fully engaged member of the Occupational Therapy/Physiotherapy Team providing a service within a defined area, supporting the therapists with clients who may have complex needs in a range of settings including hospital, community, patient’s homes, schools, and out-patient settings.

- Undertake intervention programmes with individual clients or groups this may involve fitting and instruction on the use of adaptive equipment and mobility aids the supervision of adaptive daily living skills and leading therapeutic groups.
- Supervise, monitor, and encourage patients undertaking rehabilitation programmes, promoting independence and engagement in rehabilitation.
- Maintain clinical records (paper & electronic) and report accurately to others.
- Carry out administrative duties in relation to clinical, service, departmental needs
- Work flexibly to meet the needs of the service, including 7-day shift pattern, with extended working hours covering 365 days of the year

### Knowledge

**Essential**

- Awareness of team dynamics and work effectively as part of a team.

**Desirable**

- Understanding of the concept of rehabilitation within the clinical area.
- Basic knowledge of some conditions and the effects of illness on function.

### Experience

**Essential**

- Ability to work single-handedly with individuals and/or groups, effective written and oral communication skills.
- Planning and organisational skills Have the ability to reflect on own performance

**Desirable**

- Computer skills e.g. use of Word, Excel, PowerPoint and data inputting

### Qualifications

**Essential**

- Quality Credit Framework qualification at level 3 e.g. NVQ or OCN related to Health & Social Care
- Computer literacy/I.T. skills and knowledge
- Documented evidence of participation in training activities

**Desirable**

- Willing to work towards Quality Credit Framework qualification at level 4 e.g. NVQ or OCN related to Health & Social Care or prepared to work towards.
- Awareness of Health & Safety legislation & practices.
