Our service offering includes telephone triage, home visiting and patients visiting one of our seven primary care centres. These services are delivered between 18:30 08:00 Monday to Friday and from 08:00 08:00 across weekends and bank holidays. Service operating times are variable based on patient demand and access to premises. The post holder will support the three commissioned services, ensuring they are safe, efficient and patient-focused. They will work closely with the Medical Lead, Clinical Lead and Business Delivery Manager and ensure patients receive optimal care in the correct setting within the healthcare system. The service is staffed by a team of GPs, Advanced Nurse/Clinical Practitioners, Nurses, Paramedics and a non-clinical workforce. Clinical Duties The GP will work with a multi-disciplinary team to ensure appropriate care is delivered in the right place, at the right time. The post holder will be expected to: Maintain the highest clinical standards for all patients accessing the service. Elucidate the primary reason for the patients referral or attendance within the OOH service. Assess and treat patients presenting with clinical needs which can be met by primary care. Provide emergency care while waiting for emergency transfer of patient when appropriate Provide advice and support to colleagues and the multi-disciplinary team. Manage the flow of patients by balancing the clinical need of the patient with the capacity of the provider to deliver the necessary response. Support both injury and illness pathways. Maintain contemporaneous medical records for all patients consulted with. Work with the Medical Lead to ensure training, significant events, complaints, audits, surveys, and other professional service development activities are achieved to ensure the smooth running of the service. Maintain continued education through attendance at courses and/or study days as necessary to ensure professional development requirements for on-going professional registration and licensing are met.
