### JOB SUMMARY

Healthcare in Prison aims to provide innovative, compassionate and patient centred care to improve the outcomes of those in Prison, with a population health focus. Working in partnership with the extensive multidisciplinary team, the post holder will use clinical pharmacy skills to improve safety, efficiency, effectiveness and consistency of prescribing within Healthcare in Prison. This may include working within the areas of primary care, mental health and addictions.

The post holder will work as an autonomous, highly skilled Practice Pharmacist within Prison Healthcare, providing clinical pharmacy, medicines optimisation and independent prescribing services to improve the quality, safety and effectiveness of care for people in prison.

The role supports multidisciplinary teams by improving prescribing practice, managing repeat prescribing systems, undertaking structured medication reviews, leading audit and quality improvement, and providing expert advice, education and professional leadership in medicines use across custodial settings.

### KEY DUTIES / RESPONSIBILITIES

### The post holder will

Deliver clinical pharmacy and medicines optimisation services within prison healthcare to improve prescribing quality, safety and patient outcomes

Work autonomously as part of the multidisciplinary team, providing expert pharmaceutical and prescribing advice

Act as an independent prescriber for agreed patient cohorts, prescribing within scope of practice, governance arrangements, professional and legal frameworks and local policy

Undertake medicines reconciliation at committal, transfer and release to support continuity of care

Manage, maintain and continually improve safe repeat‑prescribing systems, including oversight of re‑authorisation processes

Undertake timely, structured medication reviews with a focus on long‑term conditions, polypharmacy and high‑risk medicines

Assess, initiate, optimise, therapeutically switch and discontinue medicines in line with clinical need and best available evidence

Provide pharmaceutical advice in complex clinical scenarios, including mental health, substance misuse and co‑morbid long‑term conditions

Promote evidence‑based, formulary‑compliant and cost‑effective prescribing, supporting waste reduction and value for money

Use clinical systems, prescribing data and decision‑support software to identify risk, support population‑health approaches and monitor outcomes

Record and report pharmaceutical interventions in line with governance requirements

Lead or contribute to medicines‑related audit, planned audit cycles and quality‑improvement initiatives, including repeat prescribing, high‑risk medicines and polypharmacy

Support medicines‑related safety incidents and organisational medicines‑optimisation priorities

Undertake clinical management activities including physical assessment, ordering and interpreting pathology, drug monitoring and ongoing review

Refer and signpost patients to other healthcare professionals and services as appropriate

Provide education, support and advice to healthcare staff and raise awareness of good medicines management across the prison estate

Maintain professional registration, prescribing competence and continuing professional development at all times

Collaborative Working

Work as an integral member of the prison healthcare multidisciplinary team, promoting effective collaboration with GPs, nurses, non‑medical prescribers, pharmacy technicians and allied health professionals

Provide expert pharmaceutical and prescribing advice to clinicians and wider healthcare staff to support safe and effective patient care

Liaise with secondary care, mental health services, substance misuse services, community pharmacies and primary care to ensure continuity of medicines on committal, transfer and release

Influence prescribing practice to promote patient‑centred, evidence‑based and safe use of medicines

Act as a professional role model and recognised medicines expert within custodial healthcare services

Service Delivery

Develop, implement, manage and continuously improve repeat‑prescribing systems within prison healthcare to support safe and effective medicines use

Ensure robust authorisation and re‑authorisation processes, including identification of medicines due for review and systems to enable timely clinical review

Undertake medicines reconciliation within agreed timescales following reception into custody

Conduct structured medication reviews focusing on long‑term condition management, polypharmacy and deprescribing, and the safe use of high‑risk medicines

Prescribe independently for agreed patient groups in line with competence, governance arrangements and service priorities

Support safe and effective prescribing in complex clinical scenarios, including mental health, substance misuse and long‑term conditions

Implement, monitor and review clinical guidelines, formularies, standard operating procedures and care pathways related to medicines management

Support medicines‑optimisation clinics and delivery of targeted prescribing interventions in line with local service agreements

Setting Direction

Support delivery of healthcare‑in‑prison, Trust and Department of Health medicines‑optimisation priorities

Promote consistent, safe and cost‑effective prescribing practice across custodial healthcare settings

Contribute to the development, review and implementation of local, regional and national prescribing guidance

Support service redesign and continuous service‑improvement initiatives to enhance medicines use and patient outcomes

Champion high standards of pharmaceutical care within prison healthcare services.

Information / Data Management

Use clinical and prescribing systems to run searches and reports, identify patients at risk and support population‑based medicines optimisation approaches

Analyse prescribing data to identify trends, unwarranted variation, clinical risk and opportunities for improvement

Undertake audit and quality‑improvement projects, including those relating to repeat prescribing, high‑risk medicines and cost‑effectiveness

Accurately and contemporaneously record clinical activity, pharmaceutical interventions and patient outcomes

Produce reports and provide evidence‑based recommendations to support service improvement, assurance and clinical governance.

Staff Management

Deliver education and training on medicinesrelated topics to prescribers, nurses and MDT staff.

Disseminate prescribing updates, safety alerts and learning from incidents and audits.

Provide supervision, mentoring and professional support to pharmacists, pharmacy technicians and learners as required.

Support staff development and maintenance of prescribing competence.

Hold self and others accountable for maintaining professional and prescribing standards.

Quality and Governance

Contribute to and, where appropriate, lead medicinesrelated audit and quality improvement activity.

Support medicinesrelated incident reporting, investigation and implementation of learning.

Ensure compliance with medicines governance, controlled drugs legislation and local procedures.

Maintain professional registration and independent prescribing status.

Practise within legal, ethical and professional frameworks, including duty of candour and safeguarding responsibilities.
