### Job overview

- We are looking for an Allied Health Professional (PT or OT) or a nurse with passion for working with frail older adults to join our front door frailty team (OPAL) at St Georges on a one year part time secondment.
- We are a friendly, dynamic team who pride themselves on providing the best quality care for frail, older adults at St Georges.
- You will need to be used to working in a fast paced environment with  a risk positive, home first approach.

### Main duties of the job

- To be a highly visible and proactive leader acting as a clinical resource and ensuring the sustained delivery of high standards of care and service delivery
- To provide a comprehensive Geriatric assessment
- To be responsible for exploring current practice, identifying areas for development, and planning the appropriate change using current evidence to evaluate outcomes
- To play a supportive operational role and maintain a proactive approach to managing patient flow through the service
- To ensure that patients and their families are fully informed and supported by providing clinical and social support
- To assist in the development of evidence based clinical nursing/ therapist  practice in line with current research and guidelines
- To maintain clear lines of communication within the specialty team to ensure cohesive multidisciplinary management of patients
- To link with healthcare teams in order to increase knowledge and facilitate the care of this group of people in the community.
- To Deputise for the Team Lead when required
- To maintain daily accurate records of OPAL activity on Database

- Ability to use IT data bases computer literate, proven ability to use word, excel & email

- Models excellent clinical skills and is able to implement these in practice
- Demonstrates excellent verbal, written and interpersonal skills
- Able to provide and receive highly complex, sensitive and contentious information
- Ability to co-ordinate and work across professional teams and boundaries
- Able to complete a comprehensive geriatric assessment
- Knowledge and experience of completing functional and care needs assessment for frail, older adults and able to make recommendations to facilitate discharge from hospital

### Knowledge and Experience

**Essential**

- Specialist knowledge of frailty and geriatric medicine, including dementia and safeguarding
- Worked in an elderly care setting within acute or community settings as a band 6 or above
- Knowledge and experience of discharge processes within acute and community settings.
- Knowledge and experience of the mental capacity act

**Desirable**

- Worked in multiple care settings
- Experience of working in an emergency care setting
- Understanding of governance and assurance systems and application to practice

### Qualifications and training

**Essential**

- Registered Nurse or AHP (Physio or OT)
- Teaching, mentorship qualification
- Dementia Training

**Desirable**

- Leadership qualification
- Level 6 or 7 frailty study
- Skills for health frailty training
