We are looking for a Band 6 Frailty Nurse or Paramedic to join our supportive and dedicated team. This is a part-time opportunity (28.88 hours per week), offered as a 9-month fixed-term contract or secondment. In this role, you will deliver high-quality, short-term care programmes for adults referred to the Frailty Hospital at Home service across West Hertfordshire. You will provide care in patients’ own homes, supporting those with acute health needs, helping to prevent unnecessary hospital admissions, and enabling timely discharge from urgent care settings. You will use your specialist skills and knowledge to holistically assess needs and provide clinical treatments based on evidence-based practice. To supplement generic health services when acute, or concentrated nursing care and/or therapy intervention is required.
