CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

We are looking for a Band 6 Frailty Nurse or Paramedic to join our supportive and dedicated team. This is a part-time opportunity (28.88 hours per week), offered as a 9-month fixed-term contract or secondment.

In this role, you will deliver high-quality, short-term care programmes for adults referred to the Frailty Hospital at Home service across West Hertfordshire. You will provide care in patients’ own homes, supporting those with acute health needs, helping to prevent unnecessary hospital admissions, and enabling timely discharge from urgent care settings.

You will use your specialist skills and knowledge to holistically assess needs and provide clinical treatments based on evidence-based practice. To supplement generic health services when acute, or concentrated nursing care and/or therapy intervention is required.

As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

- To assess referrals made to the service, prioritise, and respond according to priority, as appropriate both in terms of level and urgency.
- To use enhanced skills and knowledge to assess physical and psycho-social needs and develop a comprehensive and concordant plan of care with patient/carers and the wider Multidisciplinary Team as needed.
- To make risk assessments, establishing the safety or otherwise of patients’ ability to stay at home, or return home safely from another care setting, providing, and arranging necessary supportive packages.
- To provide evidenced based clinical/therapeutic interventions based on ‘best practice’, to improve health outcomes and promote choice.
- To work in close collaboration with all other community agencies including staff working within the Community Adult Health Services.

- Substantial B5 Experience
- Experience of assessing and providing quality care to people with long term conditions
- Extensive range of clinical skills and experience
- Experience of working in a MDT
- Experience of autonomous/lone working

- Experience of supervising staff and completing appraisals
