The Fracture Liaison Nurse is responsible for coordinating the assessment, investigation, and management of patients aged 50 and over who have sustained a fragility fracture, with the aim of preventing future fractures. The role involves working both autonomously and collaboratively within a multidisciplinary team to ensure timely identification, treatment, education, and follow-up of patients at risk of osteoporosis.

The post holder will contribute to service delivery, quality improvement, and national reporting requirements, while maintaining high standards of clinical care and data management. The role also requires strong IT proficiency, including the use of electronic patient records, databases, and Microsoft Office applications—particularly Excel—for data tracking, audit, and reporting purposes.

- Initiate or recommend appropriate osteoporosis treatments under Patient Group Directions (PGDs) or agreed protocols
- Provide patient-centred education on bone health, medication adherence, lifestyle modification, and fracture preventio

- Maintain accurate, timely clinical documentation and patient records using electronic systems
- Use IT systems and Microsoft Excel to manage patient databases, track outcomes, and support reporting requirements

- Demonstrate strong clinical reasoning and decision-making skills

### Essential and Desirable

**Essential**

- Highly developed communication skills and ability to convey complex information and sensitive information to others
- Demonstrates skill in teaching and supervision of students and carers
- Able to demonstrate clinical knowledge, skills and expertise on the management of patients within the Rheumatology service

**Desirable**

- Experience in taking medical histories and completing episodes of patient care
