The practitioner will work in collaboration with the patient to deliver care plans aligned to recover focused outcomes. The practitioner will have direct patient contact in the form of assessment and treatment. Our practitioners within the Focused Care Pathway will be working with specific patient groups, who are recovering through different pathways. Some will require time-limited brief psychologically informed interventions leading to discharge. The post holder should have passion for developing their clinical skills and be keen to work flexibly with a diverse caseload in a variety of settings. Only open to applicants from Hampshire & Isle of Wight NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service, Isle of Wight NHS FT or Hampshire Hospitals FT. ### Main duties of the job Manage a caseload of service users, undertaking assessment, care planning, risk management and regular reviews in line with Trust policies and recovery-focused practice. Deliver evidence-based interventions, including psychosocial and CBT-informed approaches, tailored to individual needs. Work collaboratively with service users, carers and families to promote recovery, choice, independence and personalised care. Monitor mental and physical health needs, ensuring appropriate interventions and support are provided. Participate in multidisciplinary meetings, case discussions, caseload management and the Shared Care/Duty rota. Maintain accurate records and communicate effectively with colleagues, service users and partner agencies. Provide supervision, guidance and support to support workers, junior staff and students as required. Contribute to safeguarding, risk management and quality improvement activities, escalating concerns appropriately. Work autonomously to prioritise workload while engaging in regular supervision, reflective practice and continuing professional development. Maintain professional registration, discipline-specific competencies and mandatory training requirements. The successful candidate will be a qualified Nurse, Social Worker or Occupational Therapist with excellent assessment, communication and organisational skills, and a commitment to recovery-focused, person-centred care. ### About us Hampshire and Isle of Wight Healthcare NHS Foundation Trust provides joined-up mental and physical healthcare for around two million people across our communities.
