Community Mental Health Social Worker (Flow)

Stratford Upon Avon (TBC)

The Older Adults Service work with Older Adults who have mental health needs in the Community and following an in-patient stay. The Team is multi-disciplinary consisting of OT, Nursing, Psychiatry, Psychology, Social Worker, Voluntary sector, support workers and peer workers. The service works closely with health, social care, statutory and voluntary sector partners.

Fit for the Future 10 Year Health Plan for England (2025) is the main strategic driver for change for Older adults accessing health and social care services. The role will align with expectations for moving service provision from hospitals to the community, as well as with local and national ambitions for integrated neighbourhood teams. Place-based partnerships and integrated neighbourhood teams promote a focus on prevention and integration of statutory and voluntary agencies.

### Main duties of the job

The focus of the position is to maintain patient flow whilst supporting a strengths-based approach to recovery. The role will be based in the community teams with an expectation that at least 50% of time will be spent on the Older Adult Wards. We are looking for enthusiastic, dynamic and experienced individuals who can work confidently on their own as well as being a team player. Good communication and problem-solving skills are essential, as well as knowledge of the needs of older adults. We also are looking for people who can demonstrate the Trust’s vision and values in practice, and are motivated to be involved in innovation and service development.

As a social worker in the team, we will ensure that you have clinical and professional support from a qualified Social Worker to support the maintenance of your professional registration. This support will be alongside the supervision and management that will come from the Trust giving the best of both worlds.

### Detailed job description and main responsibilities

The successful applicant will work alongside the leads and colleagues to continue to develop the service and begin to pick up a specific relevant clinical caseload, and provide evidence based interventions as well as liaising with other agencies. You will form part of the multi-disciplinary team, and be expected to voice your professional experience to support the patients who are open to the service, and other services that may need your advice or support.

As a clinician within the team, we will ensure that you have clinical and professional support from the relevant professionals to support the maintenance of your professional registration. This support will be alongside the supervision and management that will come from the Trust giving the best of both worlds.

We encourage staff development, and you will be supported throughout your supervision and appraisal process to develop your skills and reach your potential.

You must have appropriate UK professional registration.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

- Must have full driving licence and access to a motor vehicle for business use

### Experience

**Essential**

- Substantial experience in a Community Mental health care setting
- Developed specialist knowledge gained within a similar environment over a considerable period of time
- Significant experience of undertaking assessment, formulation, risk assessment and positive risk management
- Experience of supervising / overseeing staff and students

### Qualifications

**Essential**

- Relevant Professional Qualification in OT
- Current unrestricted registration with HCPC
