### Job overview

The Paediatric Diabetes multi disciplinary team provides medical, nursing, dietetic and psychology coordination and case management of children with both type 1 and type 2 diabetes. The service works alongside multi agency and specialist Paediatric Nursing Services to ensure seamless pathways of care for patients. The family support worker is a new post fully integrated into the paediatric diabetes MDT team.

A key part of the Family Support Worker role will be to guide and support the families in areas such as; social care support, referrals, financial/ benefits advice, along with signposting to appropriate community and social resources. Previous experience in children’s diabetes support work and advising on social care, referrals etc. is desirable. It is essential to have a child health, social care or equivalent and relevant qualification.

### Main duties of the job

You will attend paediatric diabetes clinics at the main hospital site to support with checking in patients and the smooth running of clinics.

You will do finger prick tests to measure their HbA1c as well as measuring height, weight and blood pressure for all the patients attending clinic.

You will attend the MDT team weekly meetings and support the nurses and staff within the team to achieve positive clinical outcomes for children, young people and their families and that these are evidenced by performance indicators such as activity data, targets and quality measures.

### Knowledge

**Essential**

- Experience of working with families in need.
- Knowledge of safeguarding practises and protocols

**Desirable**

- Knowledge of the clinical management of diabetes

### Experience

**Essential**

- Experience in assessing client and family health needs while delivering impactful health promotion interventions.
- Experience of working with children and young people with diabetes or long-term health needs.

**Desirable**

- Experience of supporting families to access and apply for entitled benefits.

### SKILLS/ABILITIES

**Essential**

- Demonstrate an ability to work effectively as part of a multidisciplinary team.
- Ability to work within a changing environment

**Desirable**

- Experience in undertaking HBA1C tests, measurement of height, weight and blood pressure in children of all age
- Good communication skills with professionals in a variety of agencies

### QUALIFICATIONS/ EDUCATION

**Essential**

- Foundation level degree or NVQ 3 level equivalent in Child Health/ Social Care qualification.

**Desirable**

- Teaching / training experience
- Level 3 Counselling course or equivalent
