Depending on experience, Long Term Chronic Disease management will also form a large part of the role as well as the mentoring of our Health Care Assistants, Assistant Practitioners, Nurse Associate Students and RGN Student Nurses.  Knowledge of the GMS contract e.g. The Quality and Outcomes Framework, will be valuable to the post. ### Detailed job description and main responsibilities Please see the Job description and Person Specification in the attachments. PLEASE NOTE WE CANNOT OFFER SPONSORSHIP FOR THIS ROLE NOT Agenda for Change ## Person Specification ### Experience **Essential** - Experience of working in accordance with the NMC Code of Professional Conduct - Experience of dealing with the public in a respectful and professional manner - Experience of a multi-skilled approach to Chronic Disease Management - Listed core nursing skills as identified in the job summary and description - Demonstrate effective communication and interpersonal skills, including confident telephone triage consultations - Knowledge and understanding of current local and national developments and initiatives in health care, quality standards and practice development - Safely assess and develop risk management procedures and protocols - Implement strategies for quality assurance and audit - Evidence of innovative practice and reflection **Desirable** - Evidence of leading a change project - Experience of quality assurance/peer review process ### Personal Attributes **Essential** - Able to use own initiative and work autonomously - Positive attitude, self-motivated - Able to manage and organise own workload with conflicting priorities - Able to work under pressure - Recognise own limitations - Proactive in the continued development of the service - Adaptable to meet service needs ### Skills and Knowledge **Essential** - Audit experience and subsequent quality improvement - Evidence of continuing professional development - Sound IT skills - Good record keeping - Evidence of developing clinical skills.
