We are delighted to offer a rare opportunity for 2 Practice Nurses to join our nursing team at Clay Cross.

- 1 x 35 hours per week Experienced Practice Nurse based at Clay Cross

### Main duties of the job

You will work collaboratively as part of our multidisciplinary sector team of nurses and clinicians to meet the needs of our patients; delivering the highest standards of patient care.  You will undertake a diverse range of nursing assessments and provide the appropriate care / treatment  according to patient needs, striving to improve standards of care and enhancing nursing services within the practice.

Depending on experience, Long Term Chronic Disease management will also form a large part of the role as well as the mentoring of our Health Care Assistants, Assistant Practitioners, Nurse Associate Students and RGN Student Nurses.  Knowledge of the GMS contract e.g. The Quality and Outcomes Framework, will be valuable to the post.

### Experience

- Experience of working in accordance with the NMC Code of Professional Conduct
- Experience of dealing with the public in a respectful and professional manner
- Experience of a multi-skilled approach to Chronic Disease Management
- Listed core nursing skills as identified in the job summary and description
- Demonstrate effective communication and interpersonal skills, including confident telephone triage consultations
- Knowledge and understanding of current local and national developments and initiatives in health care, quality standards and practice development
- Safely assess and develop risk management procedures and protocols
- Implement strategies for quality assurance and audit
- Evidence of innovative practice and reflection

**Desirable**

- Evidence of leading a change project
- Experience of quality assurance/peer review process

### Qualifications and Training

**Essential**

- Registered General Nurse
- Registered qualification level, diploma or above
- Evidence of a commitment to continued professional development
- Specialty related course at degree level (Asthma/COPD/long term conditions), or willing to work towards
- Spirometry/Feno (or working towards)
- Independent prescriber

**Desirable**

- Mentoring course
