The PCN are looking for someone to commit to 30 hours per week. Interviews will take place in early November. ### Main duties of the job Ridge Primary Care Network (PCN) is looking for a caring, compassionate Experienced General Practice Nurse with significant post-registration expertise in frailty. The Experienced General Practice Nurse will be focusing on delivering high-quality, coordinated care to residents in care homes across the Ridge locality. This will include providing care from initial history taking, clinical assessment, diagnosis, treatment, to an initial treatment plan and assistance in completing Respect forms with the care homes and ensuring they are available on Kent Medical Community Record \[KMCR\]. The post-holder work assists the nominated practice GP to ensure compliance with the PCN Care Home Direct Enhanced Service.
