### Job summary

An exciting opportunity has arisen for an enhanced specialist nurse to pilot and lead on the development of our new PCN and Neighbourhood Frailty Team. Experience of long term condition management with the associated post graduate diploma qualifications, leadership and line management of registered professionals are essential to deliver on our ambitious goals. You must hold a level 7 MA qualification relevant to the role for example, MA healthcare leadership. Initially, you will lead a team of healthcare assistants to support the collation of clinical data required to reviews long term conditions for housebound and care home patients. Completing desk top reviews, with a requirement to carry out home and care home visits as required for patient care and clinical supervision. This is a key role in leading the expansion of this service to further realise our neighbourhood ambitions.

### Main duties of the job

The post holder will provide high quality, patient centred care for frail, housebound and care home patients, undertaking home visits where clinically indicated and supporting patients with frailty, complex needs and multiple long term conditions. Duties include monitoring and reviewing care plans, identifying deteriorating patients, coordinating interventions and working with GPs, community services, social care and voluntary partners to deliver integrated care, alongside health promotion, screening, safeguarding and referral management.

The post holder will provide clinical leadership to the Neighbourhood Frailty Team, offering support, supervision and mentorship. They will undertake advanced assessments, support complex clinical decisions, lead community based interventions and promote evidence based practice and quality improvement.

As line manager, the post holder will support staff performance, wellbeing and development through supervision, audits, competency assessment and attendance management, monitoring standards and acting as a key contact for staff.

The role also involves coordinating and prioritising workloads, deploying resources, monitoring caseloads and escalating concerns. The post holder will contribute to developing PCN and neighbourhood frailty services through service improvement, governance and innovation, supporting the evolution of integrated neighbourhood care.

- Asthma and COPD
- CVD and Heart Failure
- Diabetes
- Urinalysis
- ECGs
- Venepuncture
- Immunisations

**Desirable**

- Experience of Clinical Leadership of MDTs.

### Qualifications

**Essential**

- Registered General Nurse
- Professional and specialist knowledge in primary healthcare acquired through post graduate degree/diploma
- Diplomas in chronic disease management e.g. Diabetes, asthma, COPD etc
- A level 7 Qualification relevant to the role, e.g. MA Leadership Qualification or other MA level qualification relevant to the role.
- This is essential for Group F roles

**Desirable**

- Diabetes PgDip is desired.
- Non-Medical Prescriber Qualification.
