Due to secondment of the current post holder, we are pleased to offer an exciting 18‑month secondment opportunity for an End-of-Life Care Education Facilitator.

We invite applications from experienced, motivated individuals who are passionate about improving end of life care through high‑quality education and workforce development.

Supporting ward and day‑care teams in delivering excellent end of life care.

You will also work with the palliative care lead nurses at each site to develop learning and teaching strategies that reflect the needs of staff, patients and caregivers.

### Main duties of the job

This unique role combines clinical, educational and audit skills across two diverse hospital sites within Barts Health: the specialist centre at St Bartholomew’s Hospital and the district general setting at Newham Hospital, both serving the diverse population of East London and beyond.

Working closely with the palliative care, end of life care education and wider multidisciplinary teams, the post holder will play a key role in transforming end of life care, supporting patients and carers by developing staff through targeted learning opportunities aligned with evidence‑based practice. You will demonstrate excellent communication skills, supporting staff to engage confidently in end-of-life conversations with patients and families.

As a visible role model, you will demonstrate strategies that enhance the experience of patients and carers at the end of life, consistently embracing our We Care values. You will work collaboratively across the Trust to influence positive experiences for those approaching end of life.

The role includes promoting safe, competent and evidence‑based practice, contributing to the development of policies, processes and SOPs, and tailoring teaching to the needs of specific clinical areas. You will work independently and across teams to ensure the continued development of high‑quality care.

This full-time role is spilt 0.5 / 0.5 WTE between the Bart’s and Newham sites.

### Skills

**Essential**

- Patient focused
- Good communication and interpersonal skills
- Capable of dealing with sensitive and distressing situations
- Able to use initiative and prioritise patient need
- Caring and compassionate
- Ability to teach and adapt styles to meet educational needs of different learners from wide range of disciplines
- Ability to work as an independent autonomous practitioner

### Knowledge

**Essential**

- An in depth understanding of clinical issues relating to end-of-life care patients
- Broad understanding of treatment options, the compassionate care plan, and advanced care planning
- Knowledge of national and local strategies and NHS cancer plan in relation to end of life care.
- endocrine cancer pathways
- Knowledge on care and management of T34 syringe drivers

**Desirable**

- Willingness to undertake training courses specific to chosen clinical area

### Experience

**Essential**

- Has worked in cancer care or related speciality with exposure to end of life care for minimum 2 years' experience as a band 6
- Good working knowledge of symptom management and treatment in end-of-life care
- Experience of nursing practice in accordance with the nursing and midwifery council code of conduct in an acute setting
- Experience in managing and leading clinical team / service
- Experience in audit

**Desirable**

- Previous experience working as a CNS or clinical nurse educator
- Breaking bad news.
- Understanding of quality improvement (QI)

### Qualifications

**Essential**

- Effective Registration with the Nursing & Midwifery Council (RN).
- Degree Qualification in Nursing,
- Nurse Education Qualification
- Any communications course qualification
- Experience in caring for patients receiving end of life care

**Desirable**

- Advanced Clinical Practice
- Physical assessment module
