ED Patient Flow Coordinator Band 3, fixed term for 9 months

You will join a team dedicated to the following:

- Improve 4 hour performance
- Monitor patient flow
- Coordinate investigations, admissions, and discharges and assisting in the preparation for transfers to back of house wards and other hospitals.
- Liaise with site team, bed managers, and specialty teams
- Escalate delays early
- Release NIC from administrative tasks

### Main duties of the job

- To assist with the flow  throughout the Emergency Department.
- Escalate delays to the Nurse/ Clinician in charge (NIC/ EPIC)
- Identify all patients at 3 hours within A&E department, liaising with relevant staff to ensure a plan is in place to facilitate transfer / discharge from A&E to achieve 4 hour targets.
- Early escalation of delays
- Maintain awareness and implement escalation plan as required.
- Ensure notes are copied and ready for transfer.
- To facilitate transfers throughout the hospital
- To facilitate transfers to other hospitals
- Answering of telephone and relaying messages
- To ensure correct data entry on symphony system.
- Accurate breach recording
- To work alongside nurse in charge to ensure smooth running of the department.
- Communicate with the NIC/ EPIC, Clinicians, Site team, Specialities, radiology
- Arrange transport
- Assist with discharge planning from onset of admission, working with members of the multidisciplinary team to ensure timely and safe discharge.
- Advise PTS / ambulance control of transport requirements or when problems arise with transport for discharges.
