### Job overview

To be responsible for the review of investigation and treatment codes inputted by Clinical staff into clinical documentation.

To be responsible for updating any omissions onto the EPR system for each patient

To closely follow the Standard Operating Procedure for this process

To work autonomously when updating EPR making decisions on what

should be recorded

should all data fields in investigation / treatments have already been

populated this requires review of multiple IT Systems .

To work as a member of a small team ensuring excellent team

communication at all times.

To contribute to the production of reports as required demonstrating the on-going results of the coding

review.

To adhere to adhere to all policies regarding Confidentiality by ensuring the data handled within the

department is dealt with to the highest standards of confidentiality and security.

To participate in the annual appraisal process.

Be aware of the Data Protection Act 1998, Caldicott and Freedom of Information Act 2002 and their principles.

Comply with the Walsall Healthcare NHS Trust Policies and Procedures and departmental Policies and Procedures with regard to confidentiality,

ensuring that all information handled within the department is dealt with to the highest standard of confidentiality and security,

and to respond in a professional and sensitive manner when

dealing with confidential issues.

### Main duties of the job

- To be responsible for the review of investigation and treatment codes inputted by Clinical staff onto ED CAREFLOW with the handwritten clinical documentation that has been scanned into ED CAREFLOW.
- To be responsible for updating any omissions onto the ED CAREFLOW system for each patient
- To closely follow the Standard Operating Procedure for this process to ensure data is recorded, stored and archived appropriately.
- To work autonomously when updating ED CAREFLOW making decisions on what should be recorded    should all data fields in investigation / treatments have already been populated
- This requires review of multiple IT Systems and the collation of these systems into a unified report.
- To work as part of a small team always ensuring excellent team communication.
- To adhere to all policies regarding Confidentiality by ensuring the data handled within the department is dealt with to the highest standards of confidentiality and security.

**Essential**

- Frequent sitting in a restricted and inputting onto a computer for long Periods of time using dual computer monitors
- To work over a 5-day period.
- Display a flexible approach to work and the ability to cope with changes in environment and new technology.

### Qualifications

**Essential**

- 4 GCSE’s Grade C+ or equivalent
- Good general standard of education

### Experience / Skills

**Desirable**

- Excellent computer skills.
- To be trained in: ED ED CAREFLOW Coding System & Coding procedures.
- Good working knowledge of Word; Office Systems,
- Ability to prioritise and organise workload effectively.
- Ability to interpret Clinical records to identify omissions based on Guidance.
- Ability to make data entry details based on guidance
- Knowledge of Medical terminology, anatomical and physiological terms, Through work-based training

### Communication Skills

**Desirable**

- Excellent oral communication skills to be able to communicate face to face and on the telephone with all levels of staff. To provide and receive complex information and the ability to communicate these complex coding issues to medical, clinical and administrative staff along with external agencies.
- Comprehensive working knowledge of Microsoft Office software and Clinical Web Portal system and various IT clinical and financial systems. (ED CAREFLOW).
- Good understanding of Access to Medical Records Act
