This role is part of the Humber and North Yorkshire Dynamic Support Keyworker Service, which works alongside autistic children and young people, and children and young people with a learning disability, up to the age of 25, who are at an increased likelihood of experiencing a mental health hospital admission. The service provides support across Hull, East Riding of Yorkshire, North Lincolnshire, North East Lincolnshire, North Yorkshire and York. We work in partnership with individuals, families, carers and professionals across health, education, social care, and the voluntary and community sector to ensure support is coordinated, personalised and responsive to what matters most to each person.
