### Detailed job description and main responsibilities

Will be responsible for a cohort of patients that require domiciliary ventilation at home. Knowledge of their conditions, including but not exclusive to, COPD, OHS,OSA, chest wall deformity and NMD including MND. Understanding how different disease processes affect ventilation and be able to titrate pressures according to patient requirements.

Responsible for running outpatient clinics in a variety of settings including hospitals and the local hospice. Ensuring high quality care using evidence based practice and up to date knowledge to provide effective management plans.

Responsible for providing home visits to those unable to attend a clinic ensuring high quality care using evidence based practice and up to date knowledge to provide effective management plans.

Undertake training to be able to perform ABG’s & CBG’s as required as part of clinical assessments. Recognise the significance of observations made, and use them to develop and initiate nursing assessments and devise a plan of care. Where necessary liaise with the relevant MDT members to formulate ongoing care plan for example, if long term oxygen therapy is required.

Observe and support the band 7 with set up of new patients on domiciliary ventilation.

Responsible for monitoring data via cloud based services (Care Orchestrator and EveryWare) and acting accordingly on observations and escalating to senior as necessary.

- You will be expected to build professional relationships with the wider MDT including; respiratory team, sleep consultants and community based services.
- Will advise on the promotion of health and the prevention of illness
- Recognise and avoid situations that may be detrimental to the health and wellbeing of the individuals
- To ensure that patients’ receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines
- To act as advocate for patients within the area, to ensure a patient orientated approach to the delivery of care and to meet standards within the NHS Plan and other relevant guidelines

- Ability to set up dom vent under supervision from senior member of the team.
- Ability to lead a nurse led clinic.
