Band 7 - District Nursing Practice Facilitator, Caseloads and Improvement

Full time - 37.5 hours per week

The District Nursing Practice Facilitator will support the continuous improvement within district nursing teams, focusing on caseload management, strengthening clinical practice, operational effectiveness, workforce capability and patient experience. The post holder will work alongside District Nursing Team Leads and Matrons to identify challenges, co-design solutions, embed evidence-based practice and support sustainable improvements at team level.

### Main duties of the job

The role is hands on, highly visible and focused on enabling district nursing teams to deliver safe, efficient and person-centered care. The postholder will use coaching, data-driven insights and improvement methodologies to help teams develop resilience, enhance skill mix, streamline processes and reduce unwarranted variation.
