### Job summary

An exciting opportunity has arisen to join our Community Nursing team as a District Nurse team leader.

Rated OUTSTANDING for CARE by the CQC, you will be leading a team of highly skilled and compassionate nurses, nurse associates and health care assistants.

You will manage a caseload of patients, attached to 1 or 2 GP surgeries, including case management of patients identified as being at risk of hospital admissions. Provision of skilled, professional holistic assessment of health needs, relevant nursing care and health information to members of the adult community is required.

You will need to have a District Nursing qualification and experience of managing a team. You will be expected to manage patients with a range of long-term conditions and multi pathologies.

You will act as a role model, teaching and supporting staff and students on placement in the community, so will need to be a trained Practice Assessor.

You will also manage team members, developing their skills and providing feedback through 1-1 meetings and appraisal and managing sickness and capability as required. You will also be involved in audit to ensure care remains at a high quality and evidence based.

You will need to be a car driver and have access to a vehicle.

### Main duties of the job

The Team Leader works in partnership with the primary health care team, social services, private and voluntary sector, addressing health promotion priorities both locally and nationally.

Main duties of the role are:

- To provide professional leadership including line management, recruitment, supervision/ appraisal and development both qualified and unqualified staff within the Community Team.
- To provide an expert and evidenced based Nursing service to patients requiring nursing care within their own homes.
- To work autonomously and as a member of the multi-disciplinary team, developing and maintaining effective working relationships with other agencies including acute, primary and voluntary sectors.

### Knowledge

**Essential**

- Understanding of national policy governing the delivery of adults and older peoples services
- Awareness of current developments in health and social care

- Clinical Supervision and appraisal
- An up to date working knowledge of :-
- Acute, chronic and terminal illness, wound management, care of the elderly, care of the post op patient, drug interactions, aetiology and physiology of pain, pain assessment, and strategies for effective pain and associated symptom control.
- An understanding of the implications of cultural difference for service delivery
- Knowledge and understanding of audit and research

### Experience

**Essential**

- Community experience of assessment and delivery of care to people with complex needs
- Negotiating and working across organisational boundaries
- Working as part of a multi disciplinary team
- Mentoring students and other health care professionals

### Qualifications

- Registered Level 1
- District Nurse Specialist Practice Qualification
- Evidence of recent post registration, professional and portfolio development
- Degree Level education
- Nurse Prescriber V100
- Assessor/mentorship competency or willing to undertake training.
- Advanced Assessment and Clinical Decision Making

**Desirable**

- Non-medical prescriber V300
- Working towards Masters level
- Working towards Management and/or Leadership qualification

- Car driver/owner with current driving license
- Ability to work flexibly to include early and late shifts, evenings and weekends
