### Job summary

Our new Discharge to Assess (D2A) Service has been designed to help people leave hospital as soon as they are medically ready, enabling assessment, recovery and rehabilitation to take place where outcomes are often best achieved at home.

Bringing together Occupational Therapists, Physiotherapists, Social Workers, Discharge Coordinators and Care Providers, the service will deliver timely, person-centred interventions that maximise independence, reduce unnecessary hospital stays and support individuals to achieve what matters most to them. Working across health, social care and community settings, the team will create seamless pathways that place the individual at the centre of every decision, helping people regain confidence, rebuild routines and return to everyday life.

As a developing service, D2A offers a rare opportunity to help shape a new model of care from the ground up. Alongside delivering outstanding patient outcomes, team members will contribute to service development, strengthen partnerships across the island and influence how community rehabilitation is delivered for years to come.

Specialist Physiotherapist - £64,466.07 - £71,038.46

The Specialist Physiotherapist will play a key role in supporting individuals with a broad range of rehabilitation needs following discharge from hospital. Delivering specialist assessment, treatment and rehabilitation within home and community settings, you'll work closely with patients and colleagues to help people regain mobility, confidence and independence at a pivotal stage in their recovery. Alongside managing a varied caseload, you'll contribute to service improvement, quality initiatives and multidisciplinary decision making, helping establish best practice within a new and evolving service. The role also offers opportunities to provide supervision, education and leadership support to colleagues, making it ideal for clinicians who want to combine hands-on patient care with professional growth and wider service influence.

Senior II Occupational Therapist - £64,466.07 - £71,038.46

The Senior II Occupational Therapist will provide specialist assessment and intervention for individuals with complex physical, cognitive and functional needs, supporting their safe transition from hospital to home. Working autonomously and collaboratively across professional boundaries, you will develop tailored solutions that promote independence and improve quality of life. Responsibilities include prescribing rehabilitation programmes, recommending specialist equipment and environmental adaptations, supporting carers, and contributing to service development, mentorship and innovative community-based rehabilitation.

Senior I Occupational Therapist - £71,038.46 - £79,133.77

The Senior I Occupational Therapist will provide advanced clinical leadership within the D2A service, managing highly complex cases and delivering specialist assessment, rehabilitation and care planning. Acting as an expert resource across health and social care, you will influence decision-making, support integrated working and ensure individuals receive the right support at the right time. The role also offers opportunities to shape pathways, develop services and support workforce development through supervision, mentorship and professional leadership.
