Agency and Office of the Public Guardian

### Main duties of the job

Promote a smooth discharge process by concentrating on tasks which directly support and assist the multidisciplinary team with regards to safe and timely discharge of patients. This will include completing discharge planning documentation, communicating with external agencies such as Social Services, patient’s families and carers, arranging appropriate transport and liaising with all members of the Multidisciplinary Team including occupational therapy, physiotherapy, Speech and Language Therapist, Dietitian, Clinical Psychologist and Doctor.

Where appropriate provide access to patient homes to outside agencies, accompany patients to their own homes, or accompany patients to appointments with outside agencies.

Monitor and chase multidisciplinary team members and external agencies to promote timely and safe discharge to provide optimum and appropriate bed usage and reduce length of stay, and in addition manage more complex discharge issues.

Take responsibility for the issue of Discharge Notifications to Social Services to ensure reimbursement guidelines are observed

## Person Specification

### Education/Qualification

**Essential**

- Excellent communication and documentation skills evidenced through supporting information
- Demonstrate ability to learn new skills e.g. evidence of previous study
- Educated to ‘O’’ Level Standard or equivalent

**Desirable**

- Experience of multi-sectoral work
- Previous in-depth experience of Cerner or patient administration system
- Experience of Continuing Personal developmen

### Knowledge and Experience

**Essential**

- Able to create and maintain systems from which information can be produced
- Ability to write reports on discharge data and to communicate this information to all levels of clinical staff
- IT experience including extraction of data

**Desirable**

- Use of hospital IT systems such as Cerne
- Management of change (planning and implementation)
- Experience of staff management and proven leadership skills

### Skills, Abilities and Attributes

**Essential**

- Able to communicate information using a variety of written and verbal communication skills
- Proactive at problem solving using knowledge, initiative and experience
- Ability to analyse data, to notice and report trends on current discharge position

**Desirable**

- Excellent Written communication (literacy and numeracy) skills in telephone negotiation, Customer care, report writing and implementation plans
