An exciting opportunity has become available for a Discharge Co-Ordinator on D3E which is an elective surgical ward based in the Royal Gwent Hospital.

The post holder's overall purpose is to facilitate safe and timely discharges for all patients within acute beds.  The complexity of patients will vary and require differing levels of support and facilitation, however the purpose of appropriately discharging the patients to a suitable environment will remain the focus of the role.

The post holder will be required to work as part of a team but will also need to work independently when working unsociable hours, using their own initiative and problem solving skills.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

### Main duties of the job

Gather information around patients’ pre-admission ability, support mechanisms and any concerns as to whether these services will meet patients’ needs upon discharge   Collect, record, collate, and report information as directed by manager with areas associated with patient flow information   Support the ward to facilitate timely discharge   Actively contribute to daily capacity planning through managing and disseminating information relating to patient discharges and bed states

Liaise with and provide support to ward managers, patient flow team and a range of nursing, medical, therapy, social services colleagues, ambulance and other staff on a frequent basis in relation to progressing discharges     Liaise with care homes in a timely manner to reassess patients prior to discharge in accordance with CSSIW regulations   Finalise transfer arrangements to home or usual place of residence in a timely manner   Ensure all relevant information about each patient is obtained and recorded, stored and communicated maintaining confidentiality at all times   Ensure that sensitive clinical, social, personal and contentious information is handled with a high level of tact and diplomacy

Refer patients following direction of qualified staff to third sector voluntary agencies to effect a safe discharge   Review information on PSAG Boards and update accordingly   To ensure that the complex list is updated to reflect the information from the PSAG boards or information that has been verbalised or gathered

- NVQ/QCF Level 3 PLUS short courses or relevant experience to diploma level
