A unique opportunity to join an exciting project has arisen within a pilot programme on Anglesey which supports the health and wellbeing of children and families.

The successful candidate will work alongside fellow family support workers under the guidance of the Senior Project Coordinator, delivering and expanding support services to families.  This will involve engaging with families in their homes as well as the delivery of community-based interventions, using a range of agreed approaches to encourage positive changes to food, nutrition and physical activity behaviours.

The successful candidate will support the ongoing development and reach of the project through working alongside community partners using a Whole System Approach.

The post holder will work relatively independently within an agreed framework with a degree of autonomy, responsibility and accountability for tasks.   This will include responsibility for a caseload, delivery of support to groups of families and working co-productively with a range of partners and organisations in a complex community environment.

The post holder will support the project lead to monitor and quality assure interventions and contribute to the evaluation of outcomes and sustainability of approaches tested.

### Main duties of the job

Key Responsibilities:

Plan, coordinate and deliver nutrition-based 1:1 and group education sessions including practical cooking skills and community-based nutrition support for families under the direction of the project Dietitian and Senior Project Lead.

Motivate, guide and encourage families to participate in health promoting activities, monitor their progress, maintain accurate records, and report outcomes / findings to the team and /or wider partners.

Work autonomously and under own initiative on specific project areas, as agreed with the Senior Project Lead, seeking advice and support as necessary.

Develop and maintain positive partnership working across a range of stakeholders to promote the project, interventions and overall support key principles of health and wellbeing. To include communication with local authority staff, fellow health workers, leisure staff, and the voluntary sector.

Engage in partnership discussions and forums to share learnt experiences and reflect on areas of success and those for further development.

To produce resources and training materials, such as PowerPoint presentations, information sheets, displays, handouts and evaluation tools.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

### Skils

**Essential**

- Ability to work independently or as part of a team with families / community groups
- Excellent communication skills – verbal and written

**Desirable**

- Ability to speak Welsh

### Experience

**Essential**

- Previous experience of working in the NHS, social care, or community settings.
- Experience of using IT software and applications e.g. Microsoft Office, TEAMs, Outlook.
- Experience of working with children and families / relevant community groups

**Desirable**

- Experience of presenting to small groups
- Attendance of relevant courses / evidence of further training in relevant areas

### Qualifications

**Essential**

- Good general education. Evidence of sound English language skills and mathematics
- Relevant Level 3 qualification in Healthcare plus short courses / equivalent experience or willingness to work towards level 4 qualification within 3 years in post

**Desirable**

- Degree in related topic- nutrition/ health
- Food Hygiene Certificate
