An exciting opportunity has arisen for an experienced Diabetes Specialist Nurse and Independent Prescriber to join our forward-thinking Community Diabetes Team. This unique role combines advanced clinical practice with leadership of an innovative Continuous Glucose Monitoring (CGM) and insulin optimisation programme, supporting people living with diabetes to improve glycaemic control, increase independence and enhance their quality of life. Working collaboratively across primary, community and secondary care, the successful candidate will play a key role in driving service innovation, improving patient outcomes and shaping the future delivery of integrated diabetes care across Bromley.

The post holder will work collaboratively with the Diabetes Service Lead, the wider Community Diabetes Team and multidisciplinary colleagues across Bromley Healthcare and partner organisations to deliver high-quality, evidence-based diabetes care and lead service improvement initiatives.

The post holder will provide expert clinical leadership, specialist diabetes management and education for people living with diabetes, their carers and healthcare professionals. As an independent practitioner and non-medical prescriber, they will undertake comprehensive assessment, initiating, adjusting and optimising diabetes therapies within scope of practice and in accordance with local and national guidance.

The role comprises two key functions:

Diabetes Continuous Glucose Monitoring (CGM) Remote Monitoring Project Lead

You must have appropriate UK professional registration.

This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.

## Person Specification

### Experience

**Essential**

- Substantial post registration to include specialist diabetes experience.
- Experience in developing and/or delivering teaching programmes.
- Significant previous experience as a Diabetes Nurse

**Desirable**

- Experience of working in community, primary care settings.

### Qualifications

**Essential**

- Diploma/degree in nursing or equivalent experience
- Post registration Diabetes Qualification.
- V300 Nurse Prescriber

**Desirable**

- MSc in relevant topic
- DAFNE qualification
- DESMOND/X-Pert or similar qualification

### Specific Skills

**Essential**

- Up to date knowledge of current NHS Agenda.
- Excellent knowledge of diabetes and its management.
- Excellent patient management and assessment skills.
- Report writing skills.
- Teaching skills – ability to give presentations/group sessions

**Desirable**

- Experience of writing policies and procedures
- Experience of clinical audit

### Information Technology

**Essential**

- Computer literate, confident in word and e-mail.

### Professional Registration

**Essential**

- NMC Registration
