Job Summary Are you ready to make a real difference in diabetes care? We’re looking for a passionate Specialist Diabetes Practitioner to join our forward-thinking team within the Northern Care Alliance. This is a varied and rewarding position with cross-site cover—4 days at Royal Oldham Hospital and 1 day at Fairfield General Hospital—giving you the opportunity to broaden your experience and collaborate with a dynamic multidisciplinary team.

### Main duties of the job

Role Summary We are seeking a motivated Specialist Diabetes Practitioner to join our team within the Northern Care Alliance. In this autonomous role, you will deliver a high-quality, evidence-based service for people living with diabetes, working within Trust policy, national guidelines, and your professional scope of practice.

Your responsibilities include reducing inpatient bed occupancy by promoting excellent diabetes care and empowering patients to manage their condition. You will assess, plan, implement, and evaluate individualized treatment plans, ensuring appropriate care pathways and referrals. Acting as a specialist resource, you will provide expert advice to patients, families, and healthcare professionals, and deliver education and training to staff and students on all aspects of diabetes management.

You will participate in research and clinical audits, maintain compliance with NMC registration, and take accountability for your professional development through supervision and appraisal. Training opportunities will support service development and progression, including working towards or completing NMP V300.

### Skills & Abilities

**Essential**

- Ability to travel to different geographical locations across the Trust footprint in line with service needs

**Desirable**

- Full UK driving license with willingness to use own car for work purposes

### Registration and Qualifications

**Essential**

- Substantial years experience post registration
- Registered Nurse with current NMC registration (Part 1 Adult / General Level 1 or 2)
- Degree / Diploma in Nursing studies or equivalent
- Recognised teaching qualification

**Desirable**

- Non-medical prescriber or willingness to undertake
- Post basic qualification in diabetes or equivalent portfolio of evidence

### Knowledge, Training & Experience

**Essential**

- Experience of working in the field of diabetes and good clinical knowledge
- Evidence of proactive involvement in the mentorship of nurses
- Excellent interpersonal skills, evidence of development of own knowledge

**Desirable**

- Awareness of diabetes technology
