You will play a key role in improving patient outcomes by promoting best practice in diabetes care, reducing the risk of diabetes-related complications, and empowering individuals to take an active role in managing their condition. You will be responsible for assessing, planning, implementing, and evaluating personalised care and treatment plans, ensuring patients receive timely access to appropriate care pathways and specialist referrals. As a clinical expert, you will provide specialist advice and support to patients, carers, and healthcare professionals, while delivering education and training across multidisciplinary teams to enhance diabetes knowledge and skills. You will also have the opportunity to initiate and adjust injectable and oral diabetes medications, supporting patients to manage their health and wellbeing more effectively.
