### Job overview

We are looking for a highly motivated Specialist Dietitian to join our large integrated Diabetes team, including nurses, dietitians, doctors, lay educators and other healthcare staff based at the Hackney Diabetes Centre at the Homerton University Hospital.

As a successful candidate you will have had extensive experience with patients with Type 2 and Type 1 Diabetes, and women with gestational diabetes; both in individual consultations and group settings and be ready to make a major contribution to the care of patients with diabetes.

The Diabetes nurse and dietitian team together provide support to patients who attend the Hackney Diabetes Centre, inpatients in the hospital and provide a service at each GP practice in city and hackney for their diabetes patients. In addition to individual dietetic consultations, there is an emphasis on patient empowerment through structured education for patient groups with Type 1 and 2 Diabetes. We are a DAFNE and X-PERT centre and have also provide an insulin pump service to our patients with Type 1 Diabetes The service is committed to raising standards in the diabetes services provided.

We are committed to training, supervision and audit to improve clinical skills and ensure a high-quality service provision. You will take an active part in training Dietetic students and other staff.

### Main duties of the job

- To develop and provide a dietetic diabetes service to the population of City and Hackney as part of the multidisciplinary diabetes team, acting as a key dietetic resource within the team.
- To provide a dietetic service to referred out-patients and their carers attending the Hackney Diabetes Centre at Homerton Hospital and to patients referred by General Practitioners. This may include patients with T1 Diabetes, Type 2 Diabetes, ow women with gestational Diabetes.
- To provide a dietetic diabetes service in GP surgeries and Health Centres, as required.
- To deliver structured education programmes for groups of patients with Type 1 and Type 2 diabetes.
- To liaise with other hospital and community services as appropriate and to act as a resource.
- To assist in the development of standards for the dietary treatment of diabetes.
- To contribute to the development of all nutritional education materials developed for Homerton University Hospital NHS Trust patients.
- To train hospital and Primary Care staff and other Homerton University Hospital Dietetic staff in the principles of the dietary treatment of diabetes and to contribute to ensuring that a high standard of treatment is maintained across the Trust.
- To take part in audit, surveys and research projects specifically concerned with diabetes.
- To actively take part in the training of student dietitians with particular emphasis on the treatment of diabetes.
- To participate in health promotion activities.

### Experience

- Extensive experience of working in Nutrition and Dietetics and diabetes dietetics
- Experience of undertaking audit and changing practice in accordance to findings

- Understanding of team dynamics and experience of working within a team

**Desirable**

- Experience and working knowledge of insulin pumps with hybrid closed loop technology

### Education and Qualifications

- BSc Nutrition & Dietetics or relevant postgraduate qualification
- HCPC registered as a Dietitian

- Type 2 structured education - DESMOND/ X-PERT
- Type 1 Structured education - BIDAC/ DAFNE
- CSII / HCL course
- Behaviour Change training- CBT/ MI
