Working alongside the multi-disciplinary team providing and coordinating care of patients with pre-existing and gestational diabetes. To deliver woman centered advice, support and care specifically for women with pre-existing diabetes and gestational diabetes. The post holder will be expected to work closely with the diabetes multi-disciplinary team and to manage Midwife/Nurse led care clinics for women who have been identified with gestational diabetes for further assessment, education and monitoring of their condition in line with evidence-based guidelines. To work safely and effectively as an Non Medical Prescriber working within an agreed formulary.
