## Job Content

### Job overview

An exciting opportunity has arisen for a dynamic, enthusiastic and motivated Dietitian to join our award-winning Diabetes and Endocrinology Team at Mersey and West Lancashire Teaching Hospitals NHS Trust.

This is an education-focused role with the primary responsibility of delivering high-quality, evidence-based, accredited diabetes education programmes to local people with diabetes. We are looking for an individual with a passion for patient education and behaviour change who is committed to empowering people to confidently self-manage their diabetes and improve their long-term health outcomes.

The successful applicant will play a central role in promoting, developing and delivering structured diabetes education across the service. Working as part of a highly skilled multidisciplinary team, you will facilitate both group-based and individual education sessions, adapting your approach to meet the diverse cultural, educational and health literacy needs of our patient population.

The JD & PS are currently under AfC review and are subject to change.

### Main duties of the job

You will deliver nationally accredited locally delivered education programmes, including those newly diagnosed with Type 2 diabetes, those commencing insulin therapy and individuals wishing to develop carbohydrate counting skills. Follow-up may include individual consultations, telephone or virtual reviews, or group sessions to reinforce learning, assess progress, address barriers to self-management and support patients in achieving agreed health goals. You will also provide tailored dietary advice for patients with associated conditions such as hyperlipidaemia, reactive hypoglycaemia and other endocrine conditions as required.

Excellent communication, presentation and interpersonal skills are essential, as you will work closely with diabetes consultants, specialist nurses, dietitians and the wider multidisciplinary team to ensure education is fully integrated within patient care pathways. You will also liaise with primary care and community services to support seamless, person-centred care.

The role extends beyond education delivery and includes supporting patients and their families through behaviour change, self-management and signposting to appropriate community resources, including financial and food support where appropriate. You will provide culturally appropriate education, information and support that promotes healthy lifestyle choices, improves confidence and reduces health inequalities.

### Detailed job description and main responsibilities

We are seeking a creative and engaging educator who enjoys motivating others and can adapt learning to meet individual needs. A teaching qualification or experience delivering structured education programmes would be an advantage.

As part of this role you will:

- Develop, implement and evaluate specialist structured diabetes education programmes for people accessing the Diabetes and Endocrinology Department.
- Deliver engaging group education sessions and provide individual education and planned follow-up to reinforce learning, evaluate understanding and support sustained behaviour change.
- Promote self-management by supporting patients to develop the knowledge, skills and confidence to effectively manage their condition.
- Deliver culturally sensitive education tailored to individual learning needs, preferences and health literacy.
- Provide education on nutrition, diabetes management, medications, prevention of complications and healthy lifestyle behaviours.
- Work collaboratively with the multidisciplinary team to ensure education is embedded within clinical pathways and contributes to high-quality patient care.
- Support service development through audit, quality improvement, evaluation of education programmes and implementation of evidence-based practice.
- Participate in the clinical supervision of student dietitians and contribute to the education and development of colleagues where appropriate.
- Maintain up-to-date knowledge of diabetes care, structured education standards and current national guidance.

We have an excellent supervision, appraisal and professional development programme that supports staff to develop both clinically and professionally. You will be encouraged to contribute to service improvement projects, audits and service evaluation, with opportunities to present your work locally and nationally.

Our philosophy is to keep patients at the heart of everything we do. We are looking for someone who shares our commitment to delivering compassionate, high-quality care and education that makes a meaningful difference to people's lives.
