These permanent posts will see the successful candidates working alongside the District Nurse Team Leader, assisting in managing and leading a Community Nursing Team. In addition, you will work in collaboration with the wider Integrated Community Care/Primary Care services to ensure the delivery of safe, effective and patient focused planned and unplanned care at home. ### Main duties of the job - In collaboration with the District Nurse Team Leader, lead and manage the Evenings and Nights  Community Nursing Team, monitoring standards of care, delegating responsibilities and supervise team members as appropriate. - Develop and maintain appropriate communication systems. Working closely with patients/carers and families and other members of the integrated care team and statutory and voluntary agencies. - Working in partnership with other agencies, act as care manager for identified patients in order to prevent inappropriate hospital admission. - In collaboration with the wider integrated care team provide care planning, support, treatment and monitoring to patients on the general case load. - In collaboration with the Team Leader, manage resources effectively and advise on skill mix requirements for the team. - In collaboration with the Team Leader, assist with the management of team budgets. - Positively participate in and promote Clinical Governance, thus ensuring the highest quality of practice is maintained, within the service.
