### Job overview

The North London Foundation Trust (NLFT) Memory Services provide comprehensive assessment for people with memory problems and post-diagnostic support for those diagnosed with a dementia.

### Main duties of the job

The post holder will be responsible for providing compassionate, post-diagnostic support to an allocated caseload of service users and their families and carers. The focus of the role will be to support people to live well with dementia in the community by providing further information and advice, signposting to organisations that provide support, and facilitating referrals as required.

To achieve this, the post holder will need the following key skills:

- communicate effectively and work collaboratively with service users, their families and carers, GPs, Adult Social Care, and others services involved in the care and support of our service users.
- utilise a strengths based and trauma informed approach when working with service users and their families
- recognise when to escalate concerns to more senior staff
- work effectively within a multidisciplinary team

### Detailed job description and main responsibilities

- To provide advice and signposting to service users with dementia and their families and carers
- To be aware of and assist in meeting the psychological, social, physical, and spiritual needs of service users.
- To recognise and value service users as individuals, promoting and maintaining the service user’s dignity, and ensuring that the care provided respects equality and diversity and is inclusive.
- To ensure that care and support is delivered using a strengths-based, trauma informed approach.
- To support with the planning and delivery of group activity for service users, including psychosocial and therapeutic support
- To identify any safety issues in relation to the service user’s health and social care. Informing the registered practitioner/team/person in charge if there are: any significant changes in the service user’s physical or mental state, any significant factors relevant to the service user, and any incidents related to the service users.

### Skills/ Abilities

**Essential**

- Excellent verbal and written communication skills
- Ability to form and maintain effective working relationships in various team settings
- Ability to maintain boundaries and form positive therapeutic relationships with service users and carers

### Personal Qualities

**Essential**

- Motivated and enthusiastic
- Competent using computers and keyboards for entering personally generated data, generating and responding to emails, conducting web searches etc.
- Adaptable to change

### Knowledge/ Experience

**Essential**

- To demonstrate an understanding of theory and practical experience of delivering interventions to service users with dementia.
- Demonstrate a sound understanding of the needs and challenges faced by people with mental health problems
- Experience of working as part of a multi-disciplinary team of health care professionals

### Qualifications and Experience

**Essential**

- Evidence of extensive and current paid experience of working with service users who have a dementia or older adults with severe and enduring mental illness.
- Minimum of one year experience working in a mental health setting
- Educated to GCSE level or equivalent (with a minimum of 5 A – E qualifications – which must include Maths and English)
