This is a unique opportunity to play a pivotal role in delivering high-quality, proactive asthma care across neighbourhoods, bringing together healthcare professionals, schools, families and community partners to improve outcomes and reduce health inequalities. We deliver accessible, joined-up asthma care for children and young people, working across health, education, social care and community services. By bringing professionals together around the needs of families, we provide holistic, preventative support that improves asthma outcomes, promotes self-management and reduces health inequalities. ### Main duties of the job As a key member of the Integrated Neighbourhood Team, you will: - Deliver specialist asthma assessment, education and management for children and young people - Manage a caseload of patients with complex respiratory needs - Support asthma reviews within schools, clinics, community venues and family homes - Work collaboratively with GPs, paediatricians, pharmacists, school nurses, health visitors, social workers, family support services, education colleagues and the wider multidisciplinary team - Participate in multidisciplinary team meetings and neighbourhood-based care planning discussions - Contribute to integrated care plans that consider both health and wider social determinants of health - Provide advice and support to professionals across health, education and social care settings - Lead on asthma education for children, families and professionals - Support quality improvement initiatives, population health management and service development - Contribute to reducing health inequalities and improving access to care - Use population health data to identify and support children at risk of poor asthma outcomes - Act as an advocate for children, young people and families, ensuring their voice is central to care planning ### Detailed job description and main responsibilities This is an exciting time to join our organisation as we develop neighbourhood-based models of care that put children, young people and families at the heart of everything we do.
