CCOT is a 24/7 team supporting the Trust to safely and effectively manage our deteriorating patients. ### Main duties of the job The Critical Care Outreach Practitioner is mainly responsible for rapidly assess and manage deteriorating patients across the hospital, initiating early interventions to prevent further decline. Support safe discharge and follow-up of patients transitioned from Critical Care to ward environments. Respond to urgent referrals and escalation calls, providing expert clinical advice and hands-on support. Work collaboratively with multidisciplinary teams to ensure safe, coordinated patient care and appropriate escalation planning. Educate and support ward staff in recognising and managing acute deterioration. Contribute to patient safety, quality improvement, audit, and clinical governance processes. Maintain clear documentation and communicate effectively with patients, families, and staff.
