The Critical Care Outreach Lead Nurse is a senior specialist role responsible for leading the Critical Care Outreach service, delivering expert clinical care and operational leadership. The post holder provides advanced assessment, intervention and ongoing support for patients recently discharged from critical care and those who are acutely unwell on wards.

They will oversee the day-to-day running of the Outreach team, ensuring safe staffing, effective workload management and high standards of patient care in line with national guidance and organisational policy. The Lead Nurse is also responsible for driving service improvement through audit, research and performance monitoring, as well as leading the development and implementation of clinical guidelines and care pathways. They will support staff development, wellbeing and professional supervision, while working collaboratively with multidisciplinary teams to enhance patient outcomes and continuity of care.

This is a highly visible leadership role, contributing to strategic service development, governance, and the ongoing advancement of critical care and deteriorating patient pathways across the organisation.

## Person Specification

### Experience

**Essential**

- Experience in a senior post in critical care environment ideally in Outreach.

**Desirable**

- Experience of undertaking audits, writing reports and presenting to groups of health and social care professionals

### Language Skills

**Desirable**

- Welsh Speaker (Level 1)

### Qualifications and Knowledge

**Essential**

- Level 1 Registered Nurse (Adult)
- Current NMC Registration
- Masters level qualification in relevant area or equivalent experience
- Knowledge of NMC Codes of Practice
- Evidence of professional and education and development at an advanced level of education
- Recognised Critical Care Course.
- ILS and/or ALS
- Proven advanced clinical knowledge and skills in relation to critical care management & assessment.

**Desirable**

- Awareness of Clinical Governance
- Awareness of topical issues in health and social Care
- Previous outreach experience.
- ALS provider/ Instructor
- Teaching qualification and/or evidence of proficient teaching skills
