Working Pattern: 5 days a week from 9 AM to 5 PM

Location: Aldershot Centre for Health, Hospital Hill, Aldershot Hampshire, GU11 1AY

### Main duties of the job

- Provide clinical leadership within the crisis pathway, reporting to the Service Manager.
- Deliver timely mental health assessments for children and young people (CYP) presenting in crisis within acute hospitals and Paediatric Emergency Departments, including deliberate self-harm assessments.
- Work autonomously to assess and manage complex and high-risk cases from referral to discharge or transfer.
- Collaborate closely with CYPS Community Teams, Intensive Intervention Teams, paediatric services, and alliance partners to develop robust crisis and contingency (safety) plans.
- Coordinate care for CYP admitted to Paediatric Wards with mental health needs.
- Provide clinical advice, consultation, education, and training to Emergency Department and Paediatric Ward staff to enhance mental health knowledge and skills.
- Offer visible, credible leadership within the multidisciplinary team (MDT), supporting clinicians managing complex and challenging presentations.
- Lead on maintaining clinical standards, governance processes, and evidence-based practice.
- Develop and implement clinical guidelines, protocols, and training programmes relevant to crisis care.
- Ensure safeguarding responsibilities are upheld in line with relevant legislation and policies.
- Communicate sensitive information effectively and compassionately to CYP and families/carers.

### Detailed job description and main responsibilities

- Work as an autonomous practitioner providing specialist mental health assessments for children and adolescents presenting in crisis, including those with complex co-morbid conditions and high-risk behaviours.
- Conduct thorough risk assessments (including self-harm and suicide risk), formulate crisis and contingency plans, and implement positive risk management strategies.
- Deliver timely follow-up appointments and ensure safe discharge planning with documented care plans recorded on the electronic patient record.
- Facilitate referrals to appropriate internal and external services and ensure continuity of care.
- Coordinate care for CYP admitted to Paediatric Wards, liaising with Paediatric Liaison Nurses and medical teams.
- Collaborate closely with CYPS Community Teams during office hours to respond promptly to emergency referrals.
- Work jointly with Intensive Intervention Teams and wider alliance partners, including local authorities and emergency services, to ensure integrated crisis care.
- Manage a caseload of medium- to high-risk cases from referral through to discharge or transfer.

- Monitor, maintain, and improve standards of care, addressing clinical issues that may negatively impact service delivery.

- Supervise, mentor, and support junior staff and students, including undertaking recognised assessor training and participating in formal learner assessments (nurses only)
- Arrange and deliver training opportunities for Emergency Department and Paediatric Ward staff to enhance competencies in managing CYP mental health needs.
- Develop education programmes, learning outcomes, and competency assessments in collaboration with the Paediatric Matron.
- Lead on allocating safe and appropriate enhanced nursing care (‘specials’) for CYP when required.

## Person Specification

### Experience

**Essential**

- 3 years post qualifying experience, 1 of which must have been a year as a Band 6 in community.
- Experience working with CYP with mental health needs in the community.
- Undertaking mental health assessments, risk assessments, risk formulations, crisis and contingency planning and use of outcome measures.
- Understanding of family functioning.
- Ensure that outcome measures for the team are implemented and audited. Contribute to relevant research and ensure that interventions on the pathway are evidence based
- Be responsible for the initial assessment and subsequent facilitation of referral to other services and/or discharge with a signed care plan regarding safety and recorded on the electronic patient record.

### Qualifications

**Essential**

- Post Graduate level qualification in Nursing, Social Work or Occupational Therapy Valid driving license for use in the UK and access to a vehicle for work purposes
