### JOB SUMMARY

The post holder will work as part of a multi-disciplinary team within the Intensive Support Service of the South Eastern H&SC Trust. The post holder will provide support and time limited intervention adults with a disability who present with challenging behaviour, mental health difficulties or other complex needs and who are in need of timely intervention and support. This will include support to people who are experiencing a crisis.

The post holder will be guided and directed by clinicians (Behaviour Practitioners, Speech & Language Therapist, Occupational Therapist, Psychology, Social workers) to deliver a range of therapeutic interventions and support others understand and implement Positive Behaviour Support Plans and behaviour guidelines.

The post holder will be required to work on a rota basis (to include out of hours working) to provide a more rapid and flexible response to support Adults with a Learning Disability through particularly challenging and distressing periods. The post holder will work as part of the multidisciplinary team and support members of the community services in the application of planned interventions. The post holder will work within relevant guidelines, policies and legislation.

### KEY DUTIES / RESPONSIBILITIES

To participate in the assessment process.

With training, the post holder may be required to carry out selected formal assessments as requested by clinicians within the Service.

To assist behaviour support clinicians to plan, prepare and carry out therapy programmes on an individual and/or group basis for people with behaviours that challenge.

To provide support to individuals and their families under guidance and supervision, in accordance with established protocols, as directed by the supervising clinician.

To assist in providing a Positive behaviour support service in a range of environments including the client’s home, day centre, school or workplace as appropriate.

To provide support to individuals who are presenting in crisis including support in hospital or support to remain at home (out of hours when required).

To support the implementation of positive behaviour support plans for individual clients and their carers as directed by the supervising clinician.

Undertake direct observation of others working directly with those referred to the service. Record and report feedback on direct observations to others in the Service including clinical lead.

Use modelling to demonstrate strategies and techniques contained within individual Positive behaviour support plans to enable others to understand plans and implement them effectively.

To professionally challenge and report when others fail to follow / implement the positive behaviour support plan and / or other professional guidelines.

To collate and update client records as required.

To liaise with relevant staff across all agencies involved in the care, treatment or education of a referred client as appropriate.

To participate in training and supervision as required and in accordance with professional guidance as agreed with the supervising clinician

Assist with the development and implementation of visual communication and other systems in partnership with Behaviour Practitioners and / or Allied Health Professionals within the service.

To record and report progress of service user to the supervising clinician.

To record and report progress of families, care providers, support workers and others in implementing Positive Behaviour Support plans /behaviour guidelines / communication support plans etc.

Deliver in partnership with other team members Positive Behaviour Support Training Programmes.

Support other clinicians within the service to deliver a range of training to others within and external to the Trust.

Support families and care providers by linking them in with local community supports as appropriate.

To attend and contribute to Multi-disciplinary meetings.

To undertake data collection, analysis, and the production of reports and summaries, using IT and statistical programmes.

To undertake research activities including carrying out literature searches and reviews, data collection (e.g. data gathering from case files, semi-structured interviews and other assessment(s), setting up and maintaining computer data files, analysis of data and reporting of results, to assist the service in evidence-based practice.

### GENERAL MANAGEMENT RESPONSIBILITIES

The post holder will promote and support effective team working, fostering a culture of openness and transparency. The post holder will ensure that they take all concerns raised with them seriously and act in accordance with the South Eastern Health & Social Care Trust’s Raising Concerns Policy and their professional code of conduct, where applicable.

To participate in any monitoring and evaluation of the service as required in partnership and through discussion with the supervising clinician / line manager and / or head of service.

To advise the supervising clinician in all matters concerning work undertaken

To participate in training and supervision as required and in accordance with professional guidance as agreed with the line manager / Behaviour Practitioner.

Assists in the full range of administrative tasks required by the Team.

To assist in the induction of new team members.

There exists the potential for challenging behaviour which can take the form of physical and verbal aggression towards staff carers, and others, potential employees should be aware that such circumstances can arise and should be confident of their ability to cope with such situations. In addition, the post holder should work within their scope of practice and seek advice and guidance in an efficient and timely manner. Employees will be expected to support, adults, carers and others through such incidents under the guidance and direction of Behaviour Practitioners within the service.

To provide a flexible service, the post-holder may be required to work evenings, nights, weekends and bank holidays as necessary to meet the needs of the service.
