They will also lead and monitor the performance of the staff in the team. CAHTT is dedicated to providing timely, flexible, and person-centered interventions aimed at preventing hospital admissions and readmissions. The team focuses on supporting individuals within their communities, tailoring care to meet their unique needs. The role involves actively contributing to the continuous development and improvement of services, ensuring that effective care pathways and individualised care plans are in place to facilitate urgent and crisis care access. The post holder will champion evidence-based practices by offering clinical advice and support, serving as a role model, and delivering education and training as appropriate. ### Main duties of the job To contribute to the assessment of physiological and/or psychological functioning. To identify appropriate methods, techniques and equipment for different activities taking into account any risks.
