### Job overview

The purpose of this post is primarily to support and work alongside the Care Community Operational Manager, the Clinical Leads and the Primary Care Network (PCN) Development team as part of Cheshire East Integrated Care Partnership to ensure local development of the Care Community Model.

In order to strengthen the Care Communities, the Care Community Support Managers will be supporting the “on boarding” of core team members from partner organisations including Local Authorities and Primary Care practices in order to help the development of local priorities and initiatives and supporting the development of the wider network.

Due to the nature of the role, the postholder will need to have full access to a vehicle and be able to travel across the Care Community daily for work purposes.

The post holder will collaborate with stakeholders in order to achieve agreed objectives and deliver Care Community specific initiatives associated with improving all stages of the patient journey.

Day to day duties will be associated with implementing changes and delivering significant and sustainable improvements within the services through collaboration with stakeholder organisations.
