The Urgent Community Response Team (UCR) is a rapid assessment, diagnostic and treatment service to reduce unscheduled admissions for people with complex health needs, exacerbation of long-term conditions and frailty. Patients referred to the service are triaged as to be seen 'within 2 hours', or 'same day' for an urgent assessment. The H@H Frailty Virtual Ward supports patients who would otherwise be in hospital to receive the acute care and treatment in their own home, which prevents them from deterioration that would require an admission.
