## Job Content

### Job summary

We have expanded our award-winning Urgent Community Response and Hospital at Home Virtual Ward services across Mid and North Hampshire and are seeking experienced Consultant Practitioner or Foundation Consultant Practitioner to join our team.

This is a specialist role for advanced clinical professionals with a strong background in frailty. Whether your experience lies in Hospital at Home, acute frailty care, emergency settings or community urgent response, you will bring clinical expertise, leadership and a proven ability to manage complex care with confidence. The role is offered at Band 8C, though we also welcome those who meet most of the criteria and would benefit from structured development through our Foundation Pathway at an 8b level.

You will be part of one of the most advanced Consultant Practitioner networks in the country. Our Trust is nationally recognised for its commitment to multi-professional advanced practice, supported by a strong strategic framework, a well-connected peer community and close collaboration with NHS England, regularly contributing to national pilots and helping to inform best practice.

This is a role with real impact. You will lead complex clinical decision-making, contribute to service development, support learning across the system and champion innovation. Most importantly, you will help deliver person-centred care that transforms lives. If you are ready to use your expertise to make a meaningful difference, we would love to hear from you.

### Main duties of the job

Due to the expansion of the Hospital at home frailty Virtual Ward (H@H), we are seeking a consultant practitioner to work alongside the existing consultant practitioner and foundation consultant practitioner to provide senior clinical oversight seven days a week.

The Urgent Community Response Team (UCR) is a rapid assessment, diagnostic and treatment service to reduce unscheduled admissions for people with complex health needs, exacerbation of long-term conditions and frailty. Patients referred to the service are triaged as to be seen 'within 2 hours', or 'same day' for an urgent assessment.

The H@H Frailty Virtual Ward supports patients who would otherwise be in hospital to receive the acute care and treatment in their own home, which prevents them from deterioration that would require an admission.

The UCR and Frailty Hospital at Home Virtual Ward team operates from multiple bases across Mid and North Hampshire. The service operates 0800hrs-2000 7 days a week, 365 days a year.

### About us

Hampshire and Isle of Wight Healthcare NHS Foundation Trust provides joined-up mental and physical healthcare for around two million people across our communities. With over 13,000 staff working in the community and local hospitals, we deliver care at every stage of life, helping people live their best and healthiest lives.

Our mental health services include community-based support and early intervention in psychosis (EIP) for both adults and young people, alongside a network of specialist inpatient wards covering forensic, learning disability, eating disorder and older person's care.

We deliver extensive physical health services too, from urgent community response teams helping frail and older patients remain safely at home, to hospitals at home teams providing acute-level care in familiar surroundings. Our neurological services offer rehabilitation and treatment for conditions including Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease, Head Injury, Cerebral Palsy and Stroke. Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support.

Everything we do is underpinned by our CARE values of compassion, accountability, respect and excellence
